Services may be covered in-full or in-part by your health insurance or employee benefit plan. I am currently in-network for select national, federal ("FEP"), and Louisiana plans with the following insurance agencies only:
- Blue Cross Blue Shield PPO
- Blue Cross Blue Shield Federal/FEP
If you have a different insurance plan (e.g., Blue Cross Blue Shield HMO or POS, United Healthcare, Humana, Tricare, Medicare, Medicaid, Magellan, Value Options, Coventry, Cigna, etc.), you will need to call them to verify your benefits with an out-of-network mental health provider.
All out-of-network plan members are responsible for payment of fees in-full at the time of service.
Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
As a courtesy to you, I will either 1) file your insurance claims if I am in-network with your insurance company, or 2) provide you with the necessary forms for you to claim reimbursement for services from your insurance company.
Regardless of what is estimated as your portion, you will be responsible for the entire balance not paid by your insurance company.
